Medical Case Manager Coordinator Summary of Job:
SUMMARY:
The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population and provides ongoing case management services. Collaborates with program staff, clinicians and other stakeholders (external stakeholders including non-medical and family members and care givers) to cultivate a holistic approach. Duties include, assessing planning, implementing, monitoring and documenting and evaluating strengths to meet their needs. The Case manager may provide clinical casework and group work services to enhance the quality of patient management and maximize satisfaction and patient outcomes; will advocate for and seek services and provide interventions designed to promote health, prevent disease and address social determinants of health. Ensures that all necessary documentation and medical records are maintained for DSHS and HRSA grant funding requirements. Will occasionally provide case management services on several locations and home visits and occasionally work extended/weekend hours to meet client needs. Other duties as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Must display and set the example with excellent customer service skills, common courtesy maintaining eye contact and displaying active listening skills towards clientele, staff, funders and the general public at all times. Admission Data Collection and Assessment
  • Assesses, plans, monitors, and coordinates healthcare plan and social service needs from linkage to case management program through the implementation of case management standards and processes.
  • Promotes and utilizes the appropriate level of care for the patient and refers to other facilities and programs as needed.
  • Provides services in accordance to contract agreements and specific criteria. Serves proactively as a liaison with payer sources to explore coverage solutions (ADAP, ACA, RW, Medicaid, etc.) and secure coverage and documentation for reimbursement of claims for service.
  • Assist clients and/or caregivers in developing realistic goals for adherence to treatment regimen.
  • Assess readiness and develop a transition plan with youth clients to prepare them for adult healthcare services, self-advocacy and independent living. Planning, Implementation & Facilitation
  • Promotes optimal outcomes for the patient within the boundaries of the diagnosis and referrals given by the medical team.
  • Screens clientele and facilitate resources according to program requirements and available funds.
  • Coordinates, facilitates and participates in interdisciplinary (I.e. patient, family, medical team and payor) team meetings for continuity of care so that the client mayresume life in the community or learn to live within the limits of disability.
  • Utilize the review process and continuously evaluate the appropriate level of care with the interdisciplinary team, payor, and patient/caregiver(s).
  • Coordinates discharge plan with appropriate team and client ensuring appropriate follow-up, and documentation according to established guidelines. Advocacy
  • Serves as liaison, proactively collaborates and promotes effective communication with the health care team, payors, community agencies, and providers, legal and funding representatives to ensure continuity of care.
  • Participates in care conferences, family conferences, etc. when indicated or as needed.
  • Educates and supports the individual/family to be empowered for self-reliance and self advocacy..
  • Promotes early intervention referral for cost effective optimum care.
  • Represents the individual's best interests by assisting in finding necessary funding, offering treatment alternatives, and coordinating treatment. Monitoring & Evaluation
  • Develops and manages an assigned caseload by monitoring client care plans and updating them according to the client's current social and health situation, following Agency and Grantor guidelines to achieve self-management (graduation), when appropriate.
  • Uses appropriate auditing processes and tools to ensure care plan efficiency
  • Participates in performance improvement evaluation processes with particular emphasis on outcomes-oriented treatment.
  • Completes administrative reports reflecting caseload activities and documenting usage of agency resources as per agency, program and funding guidelines.
  • Represents the agency at staffing cases served by VAC/Westbrook Clinic and other health care or social service agencies.
  • Help patient and family through individual or group conferences to understand, accept, and follow care plan recommendations through the continuum of care.
  • Must comply with program training requirements according to VAC policies, program and funding requirements.
  • Must comply with strict informed consent and confidentiality guidelines and HIPAA regulation.
  • Must attend departmental and organizational meetings and events and mandatory trainings.
  • Must follow all agency rules and regulations including but not limited to Employee Handbook and safety rules and programmatic.
  • Other duties as required and assigned.
MINIMUM QUALIFICATIONS - EDUCATION & EXPERIENCE
  • A degree from an accredited 4-year college or university program in the fields of health, psychology, social services, mental health or related field.
  • Prefer one year of case management experience with people living with HIV, and/or persons with a history of mental illness, homelessness, or chemical dependence; or any equivalent combination of education and experience.
  • Experience in non-profit, healthcare setting preferred.
MINIMUM QUALIFICATIONS - KNOWLEDGE, SKILLS & ABILITIES
Knowledge of
HIV/AIDS
; knowledge of community resources and referrals; knowledge of OSHA regulations. Working knowledge of Microsoft Office, specifically Outlook, Word & Excel and Teams. Basic math skills to perform arithmetic calculations. Maintain a flexible schedule Monday - Friday, may occasionally require non-traditional hours, such as extended hours, evenings and weekends. Must have oral and written communication skills in English; organizational and time management skills. Ability to perform multiple tasks; good problem-solving abilities; demonstrate flexibility and people skills. Ability to work with people of diverse groups/backgrounds living with
HIV/AIDS.
Bilingual (English-Spanish) to effectively communicate with population served. Ability to safely operate a vehicle.
