Job Description

Role Overview The Case Manager drives the clinical logistics, crisis management, and stakeholder relationships required to deliver our custom, experiential behavioral health services. This role balances custom program development and direct family/client collaboration with rigorous acuity scheduling, treatment tracking, and inter-agency communication. Core Responsibilities1.
Clinical Coordination & Acuity Scheduling Acuity Scheduling Management:
Actively manage client acuity on a daily basis; strategically canceling, shifting, and optimizing direct-service sessions to meet real-time client needs.
Calendar Logistics:
Oversee complex schedule logistics for assigned clients and direct-service staff to maximize care consistency.
Custom Program Development:
Design and adapt tailored, experiential, outside-the-box treatment plans for clients.
Treatment Follow-Up:
Monitor and enforce post-session clinical follow-ups to ensure treatment goals are actively met. 2.
Meeting Facilitation & Collaboration Collaborative Meetings:
Prep for, attend, and contribute to vital internal and external touchpoints: Team Meetings (with facilitators and families)
Management Meetings & Staffing Meetings Trifecta Meetings Supervision:
Participate in routine clinical and administrative supervision. 3.
Crisis Management, Safety & On-Call Crisis Management:
Provide rapid, grounded crisis intervention and de-escalation for clients and staff.
On-Call Support:
Manage on-call and chat support at least once per month (rotating weekend/weeknight schedule).
Incident Reporting:
Handle, review, and manage all client incident reporting to maintain compliance and safety standards. 4.
Relationship Building & Compliance Stakeholder Relations:
Build and maintain deep, trust-based relationships with clients, families, county workers, and community stakeholders.
Comms Log Maintenance:
Keep meticulous, real-time updates in the internal communication log.
SOP & Review Support:
Provide guidance on internal Standard Operating Procedures (SOPs) and policy adherence; assist leadership with the client annual review process. Qualifications Proven experience in case management or clinical coordination within behavioral/mental health. Exceptional crisis management skills and the ability to pivot schedules dynamically.
