Job Description

Position Description Preston County Community Corrections Program- Case Manager Location:
Kingwood, WV 26537
Salary Range:
$35,000-$50,000 Nature of Work As part of a multidisciplinary treatment team, the Case Manager's role is to participate in the development of a comprehensive treatment plan, coordinate both medical and mental health visits, provide ongoing case management, and make referrals to community resources. Position is under the general supervision of the Program Coordinator. Examples of Work Performed Collects and enters client information into the Offender Information System (OIS) and the WV Offender Case Management System (WVOCMS), and electronic health records. Address referrals in a timely manner. Coordinate and monitor medical and mental health appointments including initial assessment appointments. Communicate with the doctors, psychiatrists, and other clinic staff regarding client progress. Obtain LS/CMI Assessment certification and administer the LS/CMI assessment to clients as needed. Assess client needs and develop aftercare plans, involving or referring to other clinics or staff where appropriate. Utilize health risk screenings to identify clint unmet needs. Coordinate with clients as needed for community services and Primary Care Provider, connecting the members with appropriate level of care. Analyze data, assess outcome measurements, and intervene appropriately. Provide ongoing case management to PCCCP clients including TRAC Alcohol Monitoring and community service project supervision when needed. Prepare reports, correspondence, and documents as necessary and maintain a complete record of contact with clients. Attend court hearings, prepare, and give testimony as required. Facilitate evidenced based group programming as needed. Perform general office duties that include organization of files, answering phones, use of fax and copy machine, typing, and sending/responding to emails. Perform drug screens. Perform any other related duties as delegated by the Program Coordinator. Minimum Qualifications Bachelor's Degree from an accredited college or university in human services, criminal justice, social work, psychology, counseling, or rehabilitation field.
Experience Case Management:
2 years preferred. Desirable Knowledge, Skills, and Abilities Excellent organizational skills. Ability to communicate professionally through oral and written communication. Ability to handle multiple tasks with efficiency. General knowledge of medical billing procedures. Knowledge of proper etiquette relating to office practices and procedures. Ability to understand and follow complex oral and written instructions. Skill in the use of a computer and various associated software such as Microsoft Office (Word, Access, Excel, and PowerPoint).
