This position will perform the duties required to develop, implement, and sustain a coordinated chronic disease care model for HIV-infected patients. Coordinate one-on-one health education. Administer and follow policies and procedures approved by the Shenandoah Community Health Board of Directors. Clinical Responsible for intake assessments, including patient history of present health status, weighing patients, current medications, allergies, and significant previous medical or psycho-social problems. Educate patients on HIV testing, compliance, treatment options, risk reduction, and prevention. Maintain supplies and educational materials. Periodically compile and report quality data as required to maintain grant eligibility. Organizational Develop and maintain patient access to interagency health support services by participating in community initiatives and collaborative activities with other community agencies. Prepare patient documents to confirm eligibility for program services. Identify patient needs and refer them to appropriate internal and external support services. Collaborate with individual patients to use available resources for identifying, documenting, and addressing their needs, ensuring they maintain proper HIV care status. Participate in procuring and monitoring grants related to HIV activities. Provide budget input and tracking, coordinating with accounting on client costs. Community Collaborate with community agencies to increase awareness of HIV care. Develop partnerships with regional resources to provide necessary supportive services for HIV patients. Clinical Information Systems Assist in the design and implementation of computer tracking templates for various programs. Communicate regularly with the Department of Health Administration and the HIV/AIDS, Hepatitis, STD & TB Administration to ensure the integrity of the program. Periodically audit CareWare to ensure accurate data entry. Reinforce program goals and objectives for HIV case management and patient education. Attend staff and department meetings as needed. Delivery System Design Assist in the design and implementation of a patient-focused delivery model. Interact with providers to ensure comprehensive care for patients within their scope of practice. Reinforce program goals and objectives for HIV case management and patient education. Decision Support Work collaboratively with providers in the development and implementation of clinical practice guidelines for HIV management within their scope of practice. Educate patients within your scope of practice.
QUALIFICATIONS
Certified Medical Assistant or Licensed Practical Nurse. Ability to function effectively within a multidisciplinary team. Skills in patient communication and education. Annual basic CPR/BLS certification.
