Quality Control - Data Abstractor 1st Shift -
Part Time Job Description:
The data abstractor is responsible for chart abstraction and data analysis for specified patient population and any other areas identified for performance improvement. This includes, but is not limited to, data collection, quality review of data, communication of data, reports, and information that would improve performance and enhance outcomes to physicians, staff, governmental agencies, administration and other appropriate parties. The following is intended to describe the general nature and level of work being performed by individuals assigned to this job by listing the major components for this position (i.e., key responsibilities and essential duties). Additionally, other duties may be assigned at the discretion of leadership to meet business, customer, and/or patient needs
Responsibilities:
Performs accurate and timely data collection Abstracts medical records in an accurate and timely manner Reviews medical records for core measures and other performance Develops systems and processes for timely and accurate data collection Develops and maintains accurate data retrieval system Encourages and facilitates team participation in decision making process by providing accurate data to facilitate decisions Analyzes data in a timely and accurate manner: Analyzes and interprets data that facilitates clinical practice patterns and is consistent with evidence[1]based medicine Performs secondary review and analysis of outliers and confirms outlier status prior to reporting Makes sound judgments based on information Identifies trends or potential problems in results and communicates to the Director and/or Performance Improvement Coordinator Generates a concurrent report of outliers by population Exports data to databases, as well as insurance companies, ensuring accurate submissions and deadlines are met Communicates orally and in writing with Director, departmental staff, hospital staff and others to affect positive changes or improvements Responds to requests for clinical information, statistics, and benchmarks from Administration, Planning and Development, and other hospital departments Ensures accuracy and integrity of all data prior to reporting Communicates with director to resolve problems/concerns, making suggestions to improve outcome processes to enhance team efficiency and productivity Participates as a team member on request
Essential Qualifications:
Bachelor's Degree Nursing Minimum of 5 years' experience
