In this vital role, you will provide essential support in a community-based care environment. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working within a fast-paced, collaborative environment that values your skills and commitment. Responsibilities Assist healthcare professionals with patient intake, vital signs measurement, and medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records). Experience with Quick MAR preferred Support patient care activities including medication and insulin administration. Monitor and record vital signs accurately while adhering to HIPAA regulations to ensure patient confidentiality. Provide compassionate assistance with activities of daily living including cooking and cleaning. Maintain detailed medical records and ensure compliance with medical terminology standards and documentation protocols. Collaborate effectively with nursing staff and other healthcare providers to deliver comprehensive patient-centered care. Skills Proven experience in assisted living facilities, nursing homes, hospitals, or home & community care environments. Strong knowledge of medical procedures including vital signs monitoring, medication administration, catheter care, and tube feeding. Familiarity with EMR/EHR systems such as Point Click Care, Quick MAR, or Matrix. Excellent understanding of medical terminology, anatomy knowledge, physiology principles and HIPAA compliance standards. Prior experience in hospice or acute care settings is a plus; working with individuals with disabilities is preferred. Strong communication skills to collaborate effectively within multidisciplinary teams and support patients' needs.
