Job Description

Medical Records Coordinator Wapakoneta Manor | Wapakoneta, OH Wapakoneta Manor is seeking a Medical Records Coordinator to join our team. This role is essential to ensuring accurate, organized, and compliant resident records within our skilled nursing facility. If you are detail-oriented, organized, and comfortable working in a healthcare environment, we want to hear from you.
Responsibilities:
Maintain and organize resident medical records (electronic and paper) Ensure records are complete, accurate, and compliant with regulations Process admissions, discharges, and transfers documentation Scan, file, and upload documents into EMR systems Respond to requests for medical records from staff, families, and outside agencies Maintain strict confidentiality in accordance with HIPAA Audit charts for completeness and follow up on missing documentation Work closely with nursing, therapy, and administrative teams ( These are core responsibilities aligned with standard medical records roles in healthcare settings )
Qualifications:
Previous experience in medical records or healthcare preferred Experience in long-term care is a plus Knowledge of EMR/EHR systems Strong organizational and time management skills Ability to maintain confidentiality and handle sensitive information Basic computer skills (Microsoft Office, data entry) High school diploma or equivalent required
