OVERVIEW

Provide intensive mental health therapy for clients with severe mental illness and co-occurring substance abuse problems. Provide intensive vocational rehabilitation for clients with severe mental illness.

REPORTS TO

ACT Program Manager

ESSENTIAL FUNCTIONS

Provide service coordination and individual support for clients. Complete treatment plans as assigned. Provide individual vocational-supportive counseling to clients. 5. Coordinate with other vocational agencies, employment sources, and educational institutions based on clients' needs. 6. Plan and provide work/school related supportive services. 7. Teach money management skills and facilitate the Financial Literacy class. 8. Provide ongoing assessment, problem solving, side-by-side services, skill training, and supervision to assist clients with ADLs. 9. Plan, structure and prompt social activities for clients. Organize and lead individual and group social activities. 10.

Advocate for individual clients:

with other providers at HopeWorks with providers from other agencies Participate in staff development training. Complete Peer reviews as requested by required dates.

OTHER RESPONSIBILITIES

Carry on-call phone when scheduled and provide crisis intervention as required. Carry out all duties in a manner that is consistent with HopeWorks mission, vision, values and policies. Duties may be changed based on agency needs. Schedule is 40 hours/week Sunday through Thursday 8am to 4pm or Tuesday through Saturday 8am to 4pm

EDUCATION, EXPERIENCEAND LICENSURE REQUIREMENTS

Meet all requirements of a Vocational Rehab specialist including: Bachelor's or Master's degree in human services related field. At least 2 years Directly Related Case Management or Substance Abuse experience and 1year experience with SDMI population. Current CPR/First aide certification or ability to obtain within 3 months of beginning employment. Current Valid driver's license and vehicle insurance.

OTHER REQUIREMENTS

Meet all requirements of a Vocational Rehab specialist demonstrating a high degree of effectiveness and independent judgment. Computer literate including skills in Word and Excel and database programs. Excellent oral and written communication skills. Good organizational and time management skills. Demonstrated ability to train, mentor and direct others. Demonstrated ability to work effectively with all internal and external constituents, including staff in other departments, vendors, clients, funders, donors and coworkers.
