Key Responsibilities:
Design and Innovate:
Create, implement, and evaluate programs tailored for outcomes management across designated patient populations.
Educate and Empower:
Provide education, advanced consultation, and management support for key service lines, ensuring your peers and patients are informed and empowered. Collaborate for
Excellence:
Team up with the patient care team to guarantee top-notch care while effectively managing clinical costs and enhancing service outcomes.
Drive Quality Improvement:
Utilize evidence-based practices to assess care pathways, uncovering opportunities for quality enhancement and cost savings.
Lead and Evaluate:
Spearhead initiatives focused on resource utilization and develop data-driven evaluations for continuous performance improvement.
Qualifications:
Requires advanced clinical skills; with appropriate credentialing based on the patient population (this is often reflected in an advance degree in the field of specialization; i . e . MSN or RRT. Requires knowledge of research/methodologies. Requires strong interpersonal; problem solving and analytical skills. Requires strong organizational skills. Requires effective communication and conflict resolution skills. Requires ability to lead and delegate effectively. Requires strong clinical expertise within assigned patient population. Requires the ability to educate patient populations and influence behaviors. Requires the ability to educate staff and serve in a mentoring role.
