This role partners with facility leadership teams to ensure high standards of patient care, survey readiness, compliance, and operational excellence. The ideal candidate is a strong clinical leader with experience in long-term care, regulatory compliance, staff development, and interdisciplinary collaboration. They are organized, detail-oriented, adaptable, and able to work independently while supporting multiple facilities and stakeholders. Key Responsibilities Meet the physical and sensory requirements stated below and be able to work in the described environment. Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment. Conduct audit to ensure appropriate documentation in EHR and soft files are in place for all events/incidents. Conduct monthly wound sweeps with the clinical leadership team to ensure wound management process is working. Provide hands-on education to stakeholders to ensure highest clinical capabilities are being delivered. Hardwires infection control practices throughout facility. Embraces technology, innovation, and participates in roll-out and hardwiring of technology. Participate in survey readiness and deep dives into all process at facility to ensure survey readiness. Completes new DON orientation focus areas. Leads QM scub with clinical leadership monthly. Coordinates with VPCO and RVP clinical plan for each facility monthly. Conduct mock surveys to assess facility's regulatory/clinical compliance and review all findings with Administrator/DON. Review facility past performance surveys and quality indicator reports prior to the pre-survey. Accurately and timely record observations during the survey regarding staff/residents interaction and/or dietary, environmental and life safety issues. Ensure that compiled findings are supported by facts 100% of the time. Conduct surveys on bi-annual basis or more often, as needed. Assist staff with development and follow-up of a plan of action to correct any identified regulatory issues. Conduct clinical compliance review and visit every four to six (4-6) weeks. Act as a liaison between Home Office and facility. Promote and contributes to extending home office mission, policies, and budgeting. Facilitate and Foster Interdisciplinary approach to resident care. Validate LOC for Medicare and Medicaid. Evaluates nursing PPD, staffing model, and scheduling process. Conduct utilization review of Medicare A and B for a sample of residents. Actively participate in assigned Facility Advisory Board and follow-up on regional assignments as an outcome of the FAB review. Written reports will be consistently completed when conducting pre-surveys or interim reviews. Required survey reports will be submitted within (5) working days after survey or interim visits are completed to Administrator, Supervisor and required Home Office Administrative Staff. Prepare Facility Visit Report on-site and review with Administrator and DON. Facility Compliance Reports and Facility Visit Reports will be submitted to Supervisor and required Home Office Administrative Staff within five (5) working days after visit. Written reports are correctly compiled according to Home Office policy and procedures. Assist facility staff with information whenever requested. Act in the role of consultant for region assigned (i.e., unusual occurrences, clinical issues, etc.). Conduct survey and interim visits, pre and post conferences with facilities in clear and concise manner. Guarantee confidential materials and information are never disclosed or made accessible to unauthorized personnel. Conduct mock surveys to assess facility's regulatory/clinical compliance and review all findings with Administrator/DON. Other special projects and duties, as assigned. Qualifications Active RN License and currently registered in state employed. Licensed health care professional (e.g., LNHA, PT, OT, SLP) may be considered. Complete the Infection Preventionist Training. On-site participation with major events and all surveys including off hours and weekends when warranted. Minimum of three (3) years related experience; supervisory experience preferred. Experienced in physical and functional adult patient assessment. Possess working knowledge of Medicare, Medicaid and Managed Care coverage, referral, and reimbursement methods. Extensive travel required, on-site visits to the facilities four (4) to five (5) days a week with a minimum of six (6) hours inside the facility working with stakeholders. Required to attend 75% of Clinical Whiteboard meetings while in facility. Effective verbal and written English communication skills. Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team environment in support of company objectives. Customer service oriented with the ability to work well under pressure. Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity. Strong analytical and problem solving skills. Ability to work with minimal supervision, take initiative and make independent decisions. Ability to deal with new tasks without the benefit of written procedures. Approachable, flexible and adaptable to change. Your Work Environment You'll work mostly indoors on floors that are tiled or carpeted. You'll be around other co-workers, residents and their guests. You'll have possible exposure to occasional slippery floors, chemicals, sharp objects, and hazardous materials and waste (including human), blood borne pathogens, and communicable diseases. You'll be around possible high-stress medical and/or life threatening situations. Physical and Sensory Requirements This role requires moderate physical activity. Standing and/or walking for more than two (2) hours per day. Sitting for more than two (2) hours at a time. Requires writing and/or computer work with repetitive typing and concentrating on computer screen and/or documents for more than two (2) hours at a time. Travel, including flying and/or driving, for more than one (1) hour at a time.
