QPCs monitor performance across contracts and by quality measure at the network and practice level, use performance data to identify opportunities for improvement, and serve as subject matter experts on HEDIS and other contractual quality measures. The role centers on educating providers and practice teams, optimizing workflows, improving documentation, and supporting quality‑driven patient care. QPCs work directly within primary care practices every month with a structured, quality‑driven agenda and ongoing follow‑up to improve quality outcomes, patient experience, and cost control. Essential Job Function Serves as the primary quality performance partner for assigned practices, owning relationship management and continuity. Coordinates touchpoints, follow‑ups, and escalations for assigned clinics in partnership with QPS and practice leadership. Servers as a liaison between the IT support for EMRs quality aspect, and Quality tools (innote, innovacer) and the clinics. Monitor overall contract performance and measure‑level performance at network and practice levels. Use performance data and dashboards to surface opportunities, risks, and trends. Support new clinic and provider onboarding for 2 full days and provide on site assistance and education. Educate practices on contractual quality expectations and how performance is calculated. Serve as subject matter expert on HEDIS and other contractual measures; answer provider and staff questions. Clarify documentation standards and workflow steps required for measure success. (Preventive care measures (e.g., control high blood pressure), Pharmacy measures (e.g., medication adherence), Patient experience measures (e.g., provider well‑informed about specialist care), Utilization measures (e.g., emergency department utilization).) Be onsite in each assigned primary care practice every month with a quality‑driven agenda. Conduct additional site visit as needed Present data including: Star rating by payer (Practice Dashboard); Member gap lists (Practice Dashboard); Agnostic performance by measure (Practice or Payer Dashboard); Performance by measure by payer (Practice Dashboard). Facilitate prioritization, action planning, and follow‑up to drive measurable improvement. Support practices use of Innovaccer dashboards to monitor performance and generate patient lists. Partner with practices to identify opportunities, set individualized quality improvement goals, and design workflows to improve care and control costs. Provide education and optimization support for Epic EMR and other EMR quality workflows: documentation standards, OPAs, health maintenance, reporting/registries. Provide guidance for other EMRs used by assigned clinics to ensure quality documentation standards are met. Support InNote and other quality‑related tools. Support practices using Innote in the independent space and assist on answering questions etc. Create or host all one note fast facts in a repository accessible by the practices Operationalize and support the Pathway to Performance Improvement (PPI) Program with THN‑affiliated practices. Create and maintain one‑page reference guides, how‑to tools, and workflow job aids. Uses established one‑pagers and training materials; escalates issues appropriately.
Education Required:
Bachelor's degree in a related field, or an additional 4 years of equivalent education and/or professional experience in lieu of a degree.
Experience Required:
Minimum of 3 years of experience in ambulatory operations, quality/performance improvement, or provider-facing consulting/education. Licensure/Certification/Listing
