Job Description

Pathways to Community is a collaborative partnership between the Coconino County Sheriff's Office and Coconino County Health and Human Services (CCHHS) that provides reentry support to individuals transitioning from the Coconino County Detention Facility back into the community. The Community Health Worker (CHW) Lead serves as the lead for Community Health Worker services and is responsible for coordinating daily CHW operations, supervising and supporting CHW staff, and ensuring timely engagement with individuals both prior to and following release from custody. This position reports to the Pathways to Community Manager and plays a critical role in fostering a collaborative, trauma-informed environment that supports successful reentry outcomes through connection to behavioral health, healthcare, housing, transportation, employment, and other community-based resources. Under general direction, is responsible for work of significant difficulty in performing professional community re-entry and community health work for the Pathways to Community Program, performing related work as assigned.

This is a limited appointment position funded through Legislative Funding SB1735 ending 06/30/2027 Performs the duties of Community Health Care Worker I and II Serves as the Community Health Worker team Lead Coordinates re-entry services for clients providing resource navigation services to improve client outcomes Leads case staffing meetings for program clients with program staff and partners Supervises and evaluates the work of Community Health Workers and facilitates training as needed Conducts training and outreach efforts in the community Identifies culturally appropriate training modalities for delivery to clients and community in response to identified community need Responsible for assigning and reviewing the work of others May be responsible for the development of additional trainings as required for the population(s) served Oversees Community Health Worker schedules, work plans, performance evaluations and disciplinary actions Acts as department liaison with various agencies providing resources Completes Community Health Worker client chart review and audits for PTC clients Performs related duties and fulfills responsibilities as assigned Essential functions of this position include but are not limited to: driving; working with and around others, co-workers, clients, agencies, etc.; working alone; dealing with interruptions; repetitive motion (hand/wrist) for computer input and driving; vision acuity (near/far); hearing and speech for ordinary conversation; touch/finger dexterity for computer work. Bachelor's degree in related field and two years' experience in a directly related role serving a community-based or vulnerable populations; OR, any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.
ADDITIONAL REQUIREMENTS
Must obtain an Arizona driver's license by date of hire and maintain it throughout employment. Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report. Fingerprint clearance may be required.
PREFERRED QUALIFICATIONS
EPICS-II (Effective Practices in Correctional Settings) training or certification. Experience working as a Community Health Worker, case manager, peer support specialist, or in a similar role serving justice-involved populations. Experience providing coordination or training support to staff or teams in a community-based setting. The ideal candidate is a compassionate, trauma-informed professional who is deeply committed to serving individuals involved in the criminal justice system and supporting their successful transition back into the community. They believe in treating every person with dignity, respect, and without judgment, and they are skilled at building trust and rapport with individuals experiencing complex and urgent needs. This candidate has experience connecting justice-involved individuals to community-based services. They are highly organized, adaptable, and able to effectively manage competing priorities in a fast-paced, high-demand environment. They are a strong communicator, an effective problem-solver, and a team player who is committed to improving outcomes for individuals reentering the community.

The challenges faced by people reentering society after incarceration and ability to develop effective programs to address these challenges Programs and services provided by governmental and non-governmental organizations that work to improve the health of the community Basic public health issues, methods, and principles Social determinants of health and engaging vulnerable populations Helping others improve their health status and quality of life Relevant public health training and educational resources to support public health goals and improve individual outcomes Business English, spelling and arithmetic, punctuation and grammar Office practices and procedures Criminal justice systems The prevalence of behavioral health and substance use and related needs of individuals Principles and practices of crisis intervention and trauma informed care Skill in: Social skills demonstrative of empathy and support Interpersonal skills, community building, and conflict mediation Delivering training and providing individual communication/coaching Utilizing Microsoft Office applications and web-based technologies Motivational interviewing Ability to: Utilize behavior management strategies Conduct assessments Adapt to individual needs of clients Understand and maintain confidentiality of client information Accept, understand, and relate to clients who have behavioral, emotional, learning, or physical disabilities Be trained in safety/security and behavior management procedures Successfully manage competing demands Work impartially with individuals of various socioeconomic, ethnic, and cultural backgrounds Work within a team environment and collaboratively to accomplish common goals and objectives.
