POSITION SUMMARY
: The Community Health Worker is responsible for providing excellent customer services while performing patient check-in and care coordination duties for NAC Mobile Health Services. The Community Health Worker (CHW) serves the community as a bridge between the members (patients) and the health care and social systems (providers, community resources) through outreach and education. The CHW is primarily responsible for providing supportive care and assistance to a diverse member population, assisting with the identification of barriers to self-care and healing, decision-making, and navigating the social and healthcare systems. The CHW provides personalized support to help members achieve good health practices, conducts community outreach, collaborates closely with an interdisciplinary team, and consistently demonstrates a high standard of ethical behavior, professionalism, compassion, and confidentiality. The CHW ensures operational and department goals are achieved and strives to improve quality and business processes for assigned areas of responsibility.
RESPONSIBILITIES
Establish and maintain trusting relationships with clients and families. Provide culturally and linguistically appropriate services to members and their families. Conduct outreach to connect clients with behavioral health, primary care, and community services. Facilitate and monitor scheduling of health and social service appointments. Assist in the development and implementation of community outreach and wellness programs. Participate in community events, health fairs, and education campaigns to promote available services. Support individualized recovery plans, crisis plans, and self-management plans. Provide one-on-one, non-clinical interventions and advocate for access to needed services. Obtain and organize referral documentation for provider review and follow-up. Perform patient check-ins, update demographics, and verify insurance or eligibility as needed. Complete timely and accurate documentation in the electronic health record (EHR) to show coordination and progress. Coordinate authorizations and referrals to outside specialists or community partners. Collaborate with care teams to identify service gaps and improve access to care. Assist clients with application for AHCCCS and follow up on applications through HeaPlus. Prepare monthly and quarterly outcome reports on outreach, engagement, and community activities. Support and actively engage in practices of inclusion through CLAS Standards. Drive the mobile health van to service locations as assigned. Ensure all mobile health van logs are completed as required. Coordinate with Facilities staff regarding maintenance and repairs for the mobile health van. Perform other duties as assigned.

EDUCATIONAL / WORK EXPERIENCE REQUIREMENTS
High School Diploma AND a minimum of five (5) years of proven work experience as a Community Health Worker or equivalent healthcare-related work experience, or; Associate degree AND a minimum of two (2) years of proven work experience as a Community Health Worker or equivalent healthcare-related work experience, or; Bachelor's degree - in social or healthcare-related degree with a strong understanding of the Community Health Worker model, or equivalent healthcare work experience required. Community Health Worker Certification required or ability to obtain within 9 months.
SKILLS REQUIREMENT
Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings Excellent verbal, written, and interpersonal communication skills; highly collaborative team approach to work and ability to work independently Familiarity with formal and informal community-based organizations and resource networks Excellent customer service skills A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges Experience in working with the Native American population preferred Must demonstrate critical thinking, problem-solving, and organizational and time management skills Must be able to work well with others in a team approach Possess and maintain a valid Arizona driver's license Fingerprint Certificates within 90 days of hire MS Office proficiency Bilingual-Spanish desired Know the economic, educational, and social problems of Native Americans and the referral sources available Requires a proficiency level typically achieved with 5 years of Intensive Case management experience. Must have a working knowledge of care management, community resources, and resource/utilization management

FAIR LABOR STANDARDS ACT
: This position is considered to be Non-Exempt and entitled to overtime pay for hours worked in excess of forty (40) hours per workweek for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws.
