Full Job Description Description:
Outreach Navigator II Multi-Disciplinary Outreach Team "Safety Sensitive Job" A Safety sensitive position is a job or position where the employee holding this position has the responsibility for his/her own safety or other people's safety. An employee has to be with clear mind and diligent while occupying such position. The Outreach Navigator II assists unsheltered individuals in connecting with and navigating resources related to housing, behavioral health, and support services. The role involves interacting with individuals who are experiencing homelessness, Serious Mental Illness, Substance Use Disorder, and other related conditions. The Outreach Navigator II is expected to work outdoors for extended periods. Duties and Responsibilities:
1. Provide outreach services to unsheltered people in the community. 2. Complete Housing Assessments with unsheltered individuals. 3. Connect unsheltered individuals to services through referrals and assistance with accessing services. 4. Assist individuals in locating temporary and permanent housing. 5. Transport and accompany participants to appointments as needed. 6. Use Evidence-Based Practices (EBPs) such as Housing First, Harm Reduction, and Motivational Interviewing, and De-escalation (CPI Model) to support individuals experiencing homelessness, Serious Mental Illness, and Substance Use Disorder. 7. Work closely and collaboratively with team members and community partners. 8. Maintain an accurate record of contacts, outcomes and other information as required. 9. Assist with coaching and training of Outreach Navigator I staff. 10. Coordinate scheduling, coverage, and review caseload assignments. 11. Other duties as assigned.
Requirements:
1. Masters or Bachelor's degree in a related field and no related experience OR; Associates' degree in a Must meet one of the following education and experience requirements: a. Master's degree or bachelor's degree in a field related to behavioral health. b. Bachelor's degree (in any field) and at least one year of full-time behavioral health work experience c. Associate's degree and at least two years of full-time behavioral health work experience. d. High school diploma or high school equivalency diploma with at least four years of full-time behavioral health work experience. 2. Demonstrated competency in Evidenced-Based Practices including: a. Housing First b. Harm Reduction c. Motivational Interviewing d. De-escalation (CPI Model) 3. Possess and maintain valid CPR/First Aid Certification, or ability to complete training and pass certification. 4. Knowledge of resources in the community. 5. Self-starter committed to a team approach, responsible, dependable, ability to set priorities, meet deadlines and work flexible hours. 6. Strong communication and interpersonal skills. 7. Strong computer skills (e.g., Microsoft Word, Outlook, Excel). 8. Valid Arizona driver's license, proof of auto insurance and reliable transportation.
Preferred Qualifications:
1. One year of experience working with the unsheltered population. 2. Experience using an Electronic Health Record (EHR). 3. Bilingual
