Position Summary:
The Health Navigator supports families through outreach, education, enrollment assistance, and care coordination related to children's health coverage and access to care. This position works collaboratively with families, community partners, healthcare providers, and the Department of Public Social Services (DPSS) to connect children and families to medical, behavioral health, and supportive services through a bidirectional referral process. The Health Navigator also supports home visiting programs by helping families overcome barriers to care and maintain continuous health coverage.
Duties and Responsibilities:
Conduct outreach and engagement to identify and assist families needing children's health coverage and healthcare services. Educate families on available health coverage programs, including Medi-Cal, Covered California, and other public benefits. Assist families with applications, enrollment, renewals, and eligibility-related coverage issues. Develop and maintain collaborative relationships with DPSS, healthcare providers, managed care plans, community organizations, and home visiting staff. Coordinate bidirectional referrals among DPSS, healthcare providers, and home visiting programs to ensure timely services and follow-up support. Support families in accessing preventive care, medical homes, dental services, behavioral health services, developmental screenings, and specialty care. Provide care coordination and navigation to reduce barriers such as transportation, language access, appointment scheduling, and understanding the healthcare system. Maintain accurate documentation of outreach, referrals, enrollment assistance, follow-up efforts, and client outcomes in required databases and reporting systems. Monitor referral outcomes and follow up with families and partner agencies to ensure successful service connections. Participate in community events, health fairs, collaborative meetings, and outreach campaigns to increase awareness of children's healthcare resources. Ensure compliance with confidentiality standards, HIPAA regulations, agency policies, and program requirements. Prepare reports and assist with data collection for program performance measures and grant deliverables. Support continuous quality improvement efforts and participate in training related to healthcare access, enrollment systems, trauma-informed care, and family engagement.
Knowledge, Skills, and Abilities:
Proficient verbal and written communication in English and Spanish. Able to work effectively both independently and collaboratively; self-starter. Knowledge of home visiting models and family-centered practices. Understanding of healthcare systems, social determinants of health, and trauma-informed approaches. Experience with referral coordination and care navigation. Ability to maintain confidentiality and handle sensitive information professionally. Strong problem-solving, advocacy, organizational, time management, and multitasking skills. Strong computer skills, including Outlook, Word, PowerPoint, Excel, internet-based applications, and Microsoft operating systems. Punctual, highly responsible, empathetic, and able to develop and maintain effective working relationships. Knowledge of Los Angeles County Department of Public Social Services and/or Department of Public Health programs, including Medi-Cal, Covered California, and county programs.
Education and Experience:
BA/BS in Social Work, Human Services, or a related field required; or an AA degree with 2 years of experience in nonprofit community-based organizations or social welfare programs. Public health coverage experience is highly desirable. Professional experience with county or community-based social services programs is highly desirable. Experience working with culturally diverse families and linking them to health and community resources.
Licensing/Certification Requirements:
Occasional driving is required. Must provide your own vehicle, valid California Driver's License and proof of full insurance coverage. Negative annual tuberculosis screen required. We require the selected candidate to undergo a criminal background check using the Live Scan fingerprinting method in compliance with applicable laws and regulations.

Compensation:
In accordance with California law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
