Job Description

Overview Join our dynamic healthcare team as a Community Health Worker specializing in Enhanced Care Management (ECM). In this vital role, you will serve as a compassionate advocate and connector for individuals with complex health needs, including those with disabilities, chronic conditions, or behavioral health challenges. Your energetic approach will empower clients to navigate their healthcare journeys, improve their quality of life, and achieve better health outcomes. Duties Build trusting relationships with clients by providing personalized support and motivational interviewing to encourage positive health behaviors. Conduct comprehensive assessments to identify clients' needs related to medical care, nutrition, addiction counseling, social services, and mental health. Develop tailored care plans that address individual goals and coordinate services across healthcare providers, social agencies, and community resources. Educate clients on medical terminology, Medicare benefits, medication management, and healthy lifestyle choices through engaging teaching methods. Collect and document data on client progress, service utilization, and health outcomes to inform ongoing care strategies. Advocate for clients by assisting with appointment scheduling, transportation arrangements, and navigating complex healthcare systems. Facilitate community outreach initiatives such as public speaking engagements to promote health education and awareness. Requirements Proven experience or background in nursing, social work, public health, or related fields; experience working with individuals with disabilities is highly valued. Strong knowledge of medical terminology, Medicare programs, nutrition principles, and addiction counseling techniques. Excellent communication skills including public speaking and motivational interviewing to effectively engage diverse populations. Ability to collect accurate data and maintain detailed records in compliance with privacy regulations. Familiarity with healthcare systems and community resources; experience in teaching or training others is a plus. Demonstrated compassion, cultural sensitivity, and the ability to work independently as well as part of a multidisciplinary team.
