Full Job Description We're Hiring:
Community Outreach Enroller Mission City Community Network (MCCN) is seeking a passionate and bilingual Community Outreach Worker and Enroller to join our mission-driven team serving the Southbay/Los Angeles Region (Inglewood, Wilshire/Koreatown). Full-time | Monday-Friday, 8:00 AM - 5:00 PM; Saturdays as needed Support the following Locations and surrounding areas:Inglewood Wilshire/Koreatown Core Responsibilities:
Conduct outreach to underserved communities about health insurance options Assist and screen patients with Covered California applications and follow-ups Provide culturally sensitive education and navigation support through Patient Portals Make follow-up calls to applicants for any missing information, access forms in the computer system, update information in the practice management system and upload application documents to each program computerized system. Maintain thorough documentation and reporting in alignment with HIPAA and program protocols Visit community locations to facilitate enrollment Build partnerships with local schools, churches, and organizations Must currently be or willing to become a Certified Enrollment Counselor to assist our patients to apply for Covered California. Provide continuous and accurate reporting of activities to supervisor. Interview patients and assist with forms and authorizations. Assess patient eligibility for various programs Provide Covered CA informational materials to potential enrollees; answer questions related to the program and invite or direct enrollees to other Covered CA educational opportunities (if applicable) Help enrollees access the Covered CA website and explain the various health plans available to the enrollees according to their specific income levels. Obtain contact information of the interest candidates whenever possible and obtain permission to contact for follow-up. Complete and submit mandated program documentation to the Program Director Notify supervisor of need to pull and file charts for eligibility documents, messages, etc. that may be subject to HIPAA Adhere to all Covered CA policies and protocols set forth by the program. Able to perform assigned duties with a minimum level of supervision and identify problems and recommend solutions. Effective Interpersonal and Group Communication; Ethically and Culturally Appropriate Customer service; Patient Advocacy Patient and Community Education on Pandemic and related health issues Outreach to patients who have difficulty in accessing health care. Train/Guide patients in using technology to access/communicate with the health providers using various platforms, including computers, tablets, and smartphones. Responsible for meeting projected enrollment goals Develop public education and information materials, presentations and displays Assist with data collection and reporting on community health trendsQualifications Required:
High School Diploma/GED, bilingual (English/Spanish), valid CA driver's license with insurance, minimum 6 months of experience in outreach or enrollmentPreferred:
Certified Enrollment Counselor (Covered CA), 6+ months experience in outreach/enrollment Strong verbal/written communication, customer service, and public speaking skills Basic-intermediate computer proficiency (32 wpm+) Knowledge of healthcare programs (Covered CA, Medi-Cal, Medicare) Flexibility, initiative, and a strong community network are key Ability to develop clear and effective outreach materials Requires the ability to independently organize field activities. Must be flexible and able to multi-task while working in a collaborative, team environment with a positive attitude Willing to travel as deemed necessary Valid California Drivers License in good standing with DMV required; current liability insuranceBenefits :
