MCC HIV Case Manager St Johns Community Health - 3.1 Los Angeles, CA Job Details Full-time $30 an hour 18 hours ago Benefits 403(b) matching Paid holidays Health insurance Dental insurance Flexible spending account Tuition reimbursement Paid time off Employee assistance program Vision insurance 403(b) Life insurance Qualifications Spanish English Grassroots mobilization Community health outreach campaigns Full Job Description The HIV Case Worker (CW) position under the general administrative direction of the Director, addresses the patient's socioeconomic needs and assists the rest of Medical Care Coordination (MCC) team with patient monitoring, following up service linkages, assisting the team in updating care plan results, following-up with patients and tracking outcomes. Additionally, the Case Worker should act as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure that new patients are enrolled in medical care seamlessly and in a timely fashion.

QUALIFICATIONS
Education & Experience Bachelor's degree in related field (human services, health services, or a similar field) (Preferred) Comfortable working with diverse populations including formerly incarcerated individuals, undocumented individuals, LGBT populations, communities of color, and those experiencing homelessness, substance abuse, and/or mental illness. Two years' related experience in a health or social service setting preferred related field. At least 2 years' experience conducting community and/or street outreach. Experience with community organizing Excellent oral/written communication, interpersonal and oral presentation skills. Bilingual - written and speech Spanish/English (required). Strong computer skills. Family Planning Health Worker Certification or similar certification (may be obtained after hire) Rapid HIV Counselor (may be obtained after hire)
RESPONSIBILITIES
Conduct outreach and presentations to underserved patient populations with particular emphasis on HIV and Sexually Transmitted Diseases/Infections (STD/STI) and related topics (included but not limited to parenting, reproductive health, contraception, men's health, communicating about sexuality within the family; and accessing SJCH services). Addresses the patient's socioeconomic needs and assists the rest of the MCC with patient monitoring, following up service linkages. Assist MCC team in updating care plan results, following-up with patients and tracking outcomes. Act as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure that new patients are enrolled in medical care seamlessly and in a timely fashion. Link patients to care and support services within and external to St. John's. Organize and maintain complete, accurate records and file reports in a timely manner. Adhere to HIPAA statutes and confidentiality of the client and policies of the lead agency. Assist with the daily functioning of the program and other general office duties.
