Since its founding in 1982, the organization has been committed to implementing health promotion, disease prevention programs, and public health initiatives that foster healthier communities.
JOB SUMMARY
Under the direct supervision of the Supervising Care Manager, the Community Health Worker (CHW) -Operations Support will deliver Community Support services and Community Health Worker services to enrollees and provide operations support for the Multipurpose Senior Services Program (MSSP) and/or Enhanced Care Management (ECM) programs as required. This role includes supporting members of the community through outreach, engagement, education, and care coordination. CHWs may deliver education, trainings, or other services as allowed under the CHW Service Benefit. CHWs will assist Lead Care Managers (LCM) with care management follow up, interpretation, translation, and other program duties as required. CHWs may also be assigned Lead Care Manager duties as appropriate. Bilingual staff are strongly preferred, along with experience working with the populations served. CHWs must be willing to work toward Community Health Worker certification within two years of hiring.
ESSENTIAL FUNCTIONS
1. Conduct outreach to potential clients, answer incoming phone calls to the main office line, perform other office or administrative duties as needed (i.e. scanning, data entry, faxing, processing forms). 2. Deliver community supports (CS) program services and Community Health Worker services to enrollees, including support related to mental health, substance use, chronic health conditions and social stability. 3. Assist Care Manager in conducting in person assessments with enrolled clients to identify medical, behavioral health, and social needs and goals. 4. Support care coordination by assisting with scheduling, follow up, and engagement for medical, behavioral health, and substance use services. 5. Provide health education and basic psychoeducation within CHW scope, including coping strategies, treatment adherence, and use of crisis resources. 6. Monitor changes in member engagement, mood, or functioning and communicate concerns to the appropriate Care Manager or supervising staff. 7. Ensure accurate, timely care recording and reporting related to outreach, engagement, and services provided. 8. Support ECM programs and staff as necessary. 9. Attend trainings, conferences, and community outreach events as required by job duties or funding sources. 10. Willingness to travel within assigned CalAIM service area as required. 11. Willingness to work occasional evenings and weekend hours to support outreach efforts and community engagement events. 12. Maintain travel logs in Outlook Calendar or other systems as directed. 13. Provide direct services to clients as needed, including health education, advocacy, engagement, and support. 14. Perform other duties as needed to support program development and sustainability
QUALIFICATION REQUIREMENTS

REQUIRED EDUCATION AND/OR EXPERIENCE
  • High school diploma or GED, plus a valid California Driver's License, reliable transportation, and current vehicle insurance.
  • At least one year of experience working in the community, including individuals facing social, complex medical, behavioral health, or substance use related challenges. Lived experience is a plus.
  • Community Health Worker certification is preferred but not required. Must be retained within 2 years of employment.
  • Ability to work independently in the field and adapt to diverse environments.
  • Strong communication, motivational interviewing, interpersonal, and organizational skills.
  • Possess the communication skills necessary to display client-centered, non-judgmental, respectful services to all clients.
  • Must be able to maintain ECM, CS, and/or CHW caseloads while ensuring ongoing engagement and documentation is compliant with all program requirements and member needs.
  • Requires a positive attitude, professionalism, and willingness to work with low-income, medically compromised individuals.
  • Proficient technical knowledge of current/prior versions of Microsoft operating systems, Office applications, Outlook/Exchange, Microsoft SharePoint.
  • Demonstrated understanding of and genuine commitment to the diverse populations we serve, as evidenced through relevant lived or professional experience.
LANGUAGE SKILLS
Fluent in English with good communication and writing skills, bilingual strongly preferred.
MATHEMATICAL SKILLS
Ability to perform basic arithmetic calculations required for tracking purchases and office tasks.
REASONING ABILITY
Ability to use good judgment to conduct professional and collaborative activities and solve practical problems within established limits and guidelines. Ability to recognize instances in which consultation with Program Supervisors, Directors, or Senior Director should be sought.
WORK ENVIRONMENT
In-office work environment. Physical demands—including bending, sitting, and driving. Travel— within the CalAIM service area as required.
