El Sol NEC's mission is for community members to reach personal empowerment to move towards self-sufficiency and contribute to their community's well-being. The Community Health Worker/Promotor de Salud advances community health by supporting individuals and families in adopting and maintaining healthy behaviors. This role partners with families while respecting diverse family structures and parenting practices. Responsibilities include administering assessment tools such as the Family Resilience and Opportunities for Growth (FROG) Scale, ASQ, ASQ-SE, and depression screenings; identifying needs; and connecting families to appropriate healthcare and supportive services. The CHW assists families in setting and achieving goals by implementing activities outlined in the Family Service Plan to address areas identified through assessments. The role also includes conducting outreach activities, accepting referrals from clinics and community-based organizations, and participating in health fairs and community events to promote available programs and services. Minimum Requirements High School Diploma or equivalent req; 2-3+ years of related/equivalent experience in a similar position with training/certifications; 80hrs. Bilingual (Spanish/ English; speak, read, and write) preferred. Receive clearance from an FBI background check (Custodian of Records for DOJ). Qualifications Minimum of 2 years of experience as a CHW, Promotor de Salud, or similar role. Willing to commute within the surrounding work area. Strong knowledge of public health principles and community resources. Excellent interpersonal, communication, and organizational skills. Strong knowledge of public health principles, community resources, and transportation equity. Basic knowledge of Microsoft Office Suite and data collection tools. Ability to work independently and as part of a team. Proven ability to multitask, prioritize tasks, and manage time effectively. Capable of working/ understanding the needs of a multicultural environment. Strong work ethics, maintaining a professional appearance, and exercising discretion with sensitive information. Ability to perform tasks related to physical activity to include sitting, standing, for long periods of time, caring, bending, moving, and lifting up to 50 lbs. Responsibilities Act as a liaison between community members and healthcare providers, ensuring individuals' perspectives are represented and their needs are addressed. Conduct a minimum of 12 home visits per week. Cultivate trust and build strong relationships with community members, healthcare organizations, and social service providers. Deliver culturally responsive health education on topics including chronic disease prevention, nutrition, and mental health. Support individuals in creating and maintaining personalized plans to adopt healthy behaviors. Inform community members about available healthcare and social resources, and assist them in effectively navigating these systems. Execute health initiatives designed to promote, maintain, and enhance individual and community health outcomes. Provide preventive services, including health screenings and vaccination education, in collaboration with healthcare teams. Gather and report data on community health trends, outreach activities, and program outcomes. Identify emerging community health needs to inform program planning and development. Receive and manage referrals from healthcare providers, social service agencies, and partner organizations. Coordinate access to medical appointments, social services, and community resources, ensuring continuity of care. Guide and mentor Community Health Workers by sharing knowledge and best practices. Contribute to the training and onboarding of new CHWs to uphold high standards of community engagement. Assist in organizing and participating in health education workshops, community meetings, and public health initiatives. Represent El Sol NEC at health-related events, promoting programs and services. Maintain accurate, secure, and confidential participant records in compliance with organizational and regulatory requirements. Participate in staff meetings, supervision sessions, and relevant trainings to support professional development. Other duties as assigned.
