Community Health Worker II Location:
San Jose, California Duration:
6 Months (Possible Extension)
Shifts:
Monday•Friday 8:00am to 5:00pm with the exception of Wednesday 10:00am to 7:00pm

Updated resume Job Summary A Community Health Worker (CHW) at Client in San Jose acts as a vital bridge between the local community and the healthcare system. They focus on outreach, care coordination, patient education, and connecting vulnerable residents to crucial medical and social services.
Key Responsibilities Patient Navigation:
Help patients navigate health plans, schedule appointments, and coordinate transportation or interpreters.
Outreach & Education:
Participate in local health fairs and community events to promote BACH services and provide resources on preventative care, nutrition, and hygiene.
Needs Assessment & Enrollment:
Screen patients to determine eligibility for public benefit programs and assist with the enrollment process.
Care Coordination:
Serve as an advocate and liaison between primary care, clinical staff, and social services (e.g., housing or food assistance).
Documentation:
Log patient interactions, surveys, and follow-ups in Electronic Health Record (EHR) systems while maintaining strict confidentiality. Required Qualifications High school diploma or GED equivalent. Experience in public benefit enrollment or completion of formal CHW certification is typically preferred. Excellent communication, a trusted presence in the local community, and proficiency with Microsoft Office. Often requires bilingual capabilities (such as Spanish or Vietnamese) to serve San Jose's diverse populations effectively. TSHL01
