Comprehensive Perinatal Health Worker (CPHW) position at Client is a full-time, frontline role focused on supporting pregnant and postpartum patients through the Comprehensive Perinatal Services Program (CPSP).
Core Job Responsibilities Case Coordination:
Provide prenatal and postpartum case management, including assessments, care plan development, and follow-up.
CPSP Compliance:
Ensure program compliance with CPSP guidelines, including billing, coding, and auditing of service units.
Patient Advocacy & Education:
Offer education to patients regarding prenatal care, nutrition, lactation support, and psychosocial needs.
Outreach & Referrals:
Conduct outreach to families, provide internal and external referrals, and act as a liaison between patients and medical providers.
Documentation:
Maintain accurate, confidential records, update databases, and compile monthly reports.
Qualifications Certification:
Requires a Family Planning Health Worker Certificate or CPHW certification (training is typically arranged by client within 6 months of hire if not already certified).
Experience:
Previous experience in community health, patient navigation, or maternal health is typically required.
Skills:
Strong communication skills, empathy, and ability to work with diverse populations. "TSHL01"
Pay:
$25.00 - $35.00 per hour
