Long-Term Assignment Job Description:
We are seeking a Bilingual Perinatal Health Worker (Spanish/English) to support pregnant and postpartum patients through the Comprehensive Perinatal Services Program (CPSP) for a leading community healthcare organization in San Jose, CA. This role focuses on patient advocacy, case management, care coordination, health education, and connecting families with essential healthcare resources. Responsibilities Provide prenatal and postpartum case management services. Conduct patient assessments and assist with individualized care plans. Educate patients regarding prenatal care, nutrition, breastfeeding, and available community resources. Coordinate referrals and follow-up care with internal and external providers. Support patient outreach and engagement activities. Maintain accurate documentation within EMR systems. Ensure compliance with CPSP guidelines and program requirements. Collaborate with healthcare providers and multidisciplinary teams. Required Qualifications Fluent in both Spanish and English . Experience in community health, patient navigation, case management, maternal health, or healthcare support services. Strong patient advocacy and communication skills. Experience working in a healthcare environment. Proficiency with EMR systems; EPIC experience preferred . Ability to work effectively with diverse patient populations. Reliable transportation and willingness to travel between San Jose and Fremont locations as needed.
Preferred Qualifications:
Medical Assistant experience. Experience supporting pregnant or postpartum patients. Family Planning Health Worker Certificate or Comprehensive Perinatal Health Worker (CPHW) certification. Knowledge of community resources and healthcare programs. TSNIT01
Pay:
Up to $32.00 per hour
