Job Description

Sowing Seeds Health is launching Pathway B, a new maternal health and nutrition initiative dedicated to improving outcomes for Black moms, pregnant individuals, and infants in San Joaquin County. We are seeking a passionate and community‑rooted Lead Care Manager to join our team. This is a full‑time position with a salary of $55,000/year, plus PTO and mileage reimbursement. Position Summary The Lead Care Manager will provide intensive, community‑based support to pregnant individuals and new parents enrolled in Pathway B. This role focuses on strengthening maternal health outcomes, reducing disparities, and supporting families through culturally grounded, relationship‑centered care. Key Responsibilities Provide direct support, care coordination, and follow‑up for program participants Conduct home visits, community visits, and telehealth check‑ins Build trusting, culturally responsive relationships with Black mothers and families Support participants in navigating healthcare, nutrition resources, and social services Maintain accurate documentation and contribute to program evaluation Collaborate with community partners, doulas, clinicians, and public health teams Participate in ongoing training, professional development, and reflective supervision Required Qualifications Experience working with Black maternal health communities or communities facing maternal health disparities Currently have a Certified Community Heath Worker Certification or ability to obtain Demonstrated understanding of Black maternal health inequities, community strengths, and culturally responsive care Lived or professional experience in maternal health, perinatal support, community health work, or related fields Strong communication skills and ability to build rapport with diverse families Ability to work independently in community settings, including travel within San Joaquin County Commitment to health equity, reproductive justice, and community‑centered care Preferred Qualifications Experience as a doula, peer counselor, lactation support provider, or community health worker Familiarity with trauma‑informed care and strengths‑based approaches Knowledge of local maternal health resources and community organizations Compensation & Benefits $55,000 annual salary Paid Time Off (PTO) Mileage reimbursement How to Apply To review the full position description and apply, please submit your resume and cover letter to HR at hope@sowingseedschc.org .
