Community Health Outreach Worker Job Type:

 Full-Time $20.00 per hour

Job Summary:

Under the direction of the Program Coordinator, the Community Health Outreach Worker will conduct outreach, and education to the community in the High Desert Region. The Community Outreach Worker will be accountable for monitoring, analyzing, and reporting activity against established goals and providing education about LISTOS and services available to the community

Duties and Responsibilities:

• Research and identify outreach locations for presentations
• Assist with planning development, and implementation of community outreach activities and referrals for health insurance.
• Collaborate with Program Coordinator to support development of an outreach calendar.
• Coordinate outreach events.
• Provide outreach presentations and distribute material (i.e. brochures, flyers) at community locations and to individuals in the community to increase disaster preparedness to the Hispanic community including farm workers, undocumented immigrants, etc.
• Accurate and timely completion of reports, and data to track number of people served.
• Adhere to all policies as set forth by CHAN.

Qualifications:

• High School Diploma or equivalent.
• 2 years work experience with social service systems/programs and community agencies. Requires own transportation, valid CA driver's license, and insurance.
• Bilingual in English and Spanish is preferred.
• Good organizational, communication and interpersonal skills.
• Outreach and Enrollment experience (preferred).
• Proficient in Microsoft Word, Excel, Powerpoint.
• Technology skill level to navigate virtual and in-person presentations.
• Ability to work independently, prioritize, meet timelines and outcomes.
• Flexibility in work hours Community Health Action Network is an Equal Employment Opportunity 32 hours per week @ $20.
