care Full Job Description Care 4 U Community Health Center is seeking a full-time, Community Health Outreach Worker (CHOW) to join our outreach team to provide services in the Maintaining outreach activities in Liberty City and surrounding communities. Join our team and help connect vulnerable individuals to life-saving healthcare, prevention, and support services. The CHOW is responsible for engaging individuals living with HIV who are not in care, as well as those at increased risk for HIV and other sexually transmitted infections (STIs). Target populations include individuals who exchange sex for money or drugs, substance users, men who have sex with men (MSM), and transgender and gender-diverse individuals.
Responsibilities include:
Conducting community outreach and recruitment for HIV/STI testing, treatment, and prevention services, including PrEP Providing HIV/STI testing, health education, risk-reduction counseling, and condom distribution Conducting social and health needs assessments Referring and linking clients to medical care, prevention services, and community resources Assisting individuals in navigating healthcare and support systems This position is primarily field-based, with approximately 90% of work performed outdoors in the community. Occasional evening and weekend hours are required. Ideal Candidate Outgoing, compassionate, and comfortable engaging strangers in conversation Enjoys working outdoors and walking for extended periods Comfortable discussing sexual health and distributing prevention materials Able to work effectively with diverse populations Comfortable conducting outreach in environments where sexual activity, substance use, or other high-risk behaviors may occur Qualifications High school diploma or GED required Valid driver's license required Strong communication and interpersonal skills Florida Department of Health HIV 501 Certification required within 90 days of hire Fluent in English and Spanish preferred Florida Board of Certification designation as Certified Community Health Worker (CCHW) preferred

$20.00 - $24.00 per hour
