Job Description

The Community Health Worker works to improve access to health care services; increase screening; understand between community members and the health and social service system; enhanced communication between community members and health providers. CHW provides social support and informal counseling, advocates for individuals and community health needs. Improve access to prenatal and postnatal health care services. Responsible for establishing trusting relationships with participants and their families while providing general support and encouragement. Effectively communicating the Perinatal and Family Center Initiative (PFCI) mission. Provide ongoing follow-up, motivational interviewing, and goal setting with participants/families. Follow-up with participants via phone calls, home visits, and visits to other settings where participants can be found. Assist participants in understanding perinatal care plans and instructions. Provide referrals for services to community agencies as appropriate. Complete required reports reflecting program services. Manage assigned caseload of participants and work collaboratively and effectively within a team. Work closely with OB-GYN medical providers and Doulas to help ensure that participants have comprehensive and coordinated care plans. Work collaboratively with other clinical personnel assigned to the same patient to communicate community resources appropriate to the needs of participants/families. Act as a patient advocate and liaison between the patient/family and community service agencies. High School diploma or GED AND Four years of progressively complex office or administrative experience.
PREFERRED QUALIFICATIONS
Preference will be given to applicants who in addition to meeting the minimum qualifications also possess the following: Have a Bachelor's degree Experience collecting and analyzing data Ability to multitask Ability to work in a team environment
