Job Description

Iowa Specialty Hospitals & Clinics is seeking a bilingual Community Health Worker (CHW) to support maternal and family health initiatives across our service area. This role focuses on improving maternal mental health awareness, increasing access to prenatal and postpartum services, and strengthening engagement with Spanish-speaking and Latino communities. Full-Time Monday - Friday 8:00am to 4:30pm Provide education on healthy pregnancy, infant care, breastfeeding, nutrition, hygiene, and postpartum recovery Support care coordination in collaboration with obstetric, behavioral health, public health, and community partners Build trust and improve communication between patients, families, and healthcare providers May conduct occasional home visits for pregnant and postpartum patients, newborns, and other high-risk individuals when appropriate and mutually agreed upon Assist with community-based outreach and education efforts
Requirements:
High school diploma or GED required; bachelor's degree in public health, nursing, health education, or related field preferred Bilingual in Spanish and English (required) Strong communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team
