Full Job Description Job Summary :
Equal Hope is a not-for-profit organization formed in response to the large racial disparity in breast cancer mortality in Chicago. Equal Hope is working to eliminate disparities in breast and cervical cancer outcomes and improve health for all Chicago area women through a multi-faceted approach that involves initiatives in policy, advocacy, outreach, education, navigation, and quality improvement in medical care. The Community Health Worker will be involved in the implementation and expansion of the Community Health program, including outreach, navigation, and other initiatives as they are developed. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.
Job Responsibilities:
1. Community Based Navigation (15% of Time). 2. Community Outreach (70% of Time). 3. Develop and maintain relationships with provider partners and community-based organizations to implement and promote Equal Hope outreach efforts and other initiatives. 4. Assist in the planning and execution of Equal Hope's annual community event, special events and presentations to reach and educate the community about the Equal Hope and its community-based initiatives. Market the Equal Hope program while attending community events to help drive program participation through initiatives with community-based groups. 5. Track all outreach activities in the navigation database. 6.
Other Duties:
(15% of Time) 7. Assist with the preparation of quarterly and bi-annual grant reports, as needed.
Required Job Qualifications:
  • Recommends two years of experience in community relations, community outreach or a related field. Candidates may substitute years of direct experience for years of education.
  • Cultural competency expressed via experience and interest working with diverse communities (race/ethnicity, economic, educational), along with the ability to work in low resource environments.
  • Genuine concern for health equity and justice, along with an interest in advocacy on behalf of the underserved.
  • Exceptional interpersonal skills.
  • Strong presentation skills.
  • Ability to use Microsoft Office, including Microsoft Access.
  • Data entry experience.
  • Flexibility to attend evening and weekend community events and meetings.
  • Amenable to change. Ability to work independently as well as integrate as part of a team and part of clinic flow. Resourceful and resilient, innovative problem solver and motivated learner.
  • Friendly, cooperative, passionate and compassionate.
  • Respects individuals' right to participate in screening and individuals'' confidentiality.
  • High School Diploma or equivalent
Preferred Job Qualifications:
  • Healthcare background.
  • Bachelor's degree
  • Experience conducting breast cancer related outreach and education.
Community Health Worker Certified Physical Demands:
  • Lifting up to 50lbs
