Under supervision, the Community Health Worker I (CHW I) provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents.Key Job Activities:
Provides supportive group, individual, and crisis interventions Provide Interpersonal skills when working with patients, including having compassion and remaining sensitive to their emotional needs when they are in extreme distress or pain Monitoring patients to ensure treatment plans are being followed and that the patient is taking all necessary medication(s) Monitors the patient's medication compliance and facilitates any scheduled medical appointments Ensuring patient safety and wellbeing while remaining dedicated to identifying ways to improve the patient's health and wellbeing Charting patient behavior and documenting any changes in behavior while ensuring that all progress notes meet documentation time guidelines on a 95% basis Willing and able to attend Supervision Sessions Participates in staff meetings, mandatory and designated in-service training sessions Ability to educate patients in group settings and providing various services in the community Ability to engage a patient therapeutically to help them learn healthy aspects of interpersonal social skills, hygiene and grooming and teamwork in social settings Perform all other duties as assignedEducation and Work Experience:
High school diploma or GED required; Associates degree and above highly preferred 5 Years of experience in leading or co-leading group and or mental health workLocation:
