Community Health Worker

Community Health Worker (CHW) The Schuyler County Health Department is seeking a full-time Community Health Worker (CHW) to join our team. This position will work with residents in Schuyler and Brown Counties to ensure they have resources necessary for healthy living. The CHW will provide outreach, education and support by assisting families with navigation of local resources and access to programs such as SNAP and WIC. Skills important for this position include compassion, attention to detail, excellent communication skills, strong organizational skills, computer skills, confidentiality. Knowledge of resources in the Schuyler and Brown County communities is preferred. Applicants must have a high school diploma or equivalent. Applicants must have or be willing to obtain the Community Health Worker certification upon employment. This certification will be completed during work hours if not currently certified. Additionally, certification as a Certified Medical Assistant is preferred but not required. The hours for this position are Tuesday through Friday from 7:30 am to 5 pm with the ability to occasionally work after hours and overnight travel. This position will be required to travel within Schuyler and Brown Counties for events, outreach and meeting with residents.
