This position works to meet the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program's focus on reduction of infant mortality, maternal mortality, low birth weight, and prematurity among high-risk birthing people. The Healthy Babies Community Liaison provides prenatal and parenting education and support to birthing people and families in Sedgwick County. Education, support and connection to referrals are conducted in clients' home and community settings. Employee will provide support during public health emergencies (Activation Level 1). Conduct home visits to support maternal and child health through assessments, education, and connection to resources. Ensure all client screenings and assessments are conducted according to program guidelines and grant deliverables. Provide education on maternal child health, safe sleep, substance use cessation, child safety, family planning, mental health, and other resources according to client needs. Complete referrals to other community agencies based on individual client needs including follow-up, and assisting clients with enrollment into health insurance services and benefits. Documentation Complete accurate and timely documentation at client visits, with full completion in 48 hours. Audit documentation, revise, and complete by deadline given. Maintain accurate documentation per program policies Outreach and Collaboration Participate in a minimum of 3 outreach events per year on weekends or evenings Promote the Healthy Babies program through presentations to community agencies and coalitions to increase community awareness and promote referrals to the program Work collaboratively with other areas of the Health Department, as well as other community agencies and coalitions, to ensure appropriate and complete wraparound services for our clients, including attending two or more coalition meetings per year. Assist with quality improvement processes within the agency. Participate in staff meetings, quality assurance activities, and appropriate in-service training.
Minimum Qualifications:
Experience and Education:
Bachelor's degree in social sciences or a related field. One (1) year experience in case management, home visiting and/or customer service, or any combination of training and experience. Education may be substituted for experience, with one year of education for one year of experience. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance. Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment.
Preferred Qualifications:
Five (5) years case management, home visiting, and/or public health experience. Data entry, evaluation and/or grant experience. Master's in Social Work or related field. Current License in Social Work in the state of Kansas. Attends required trainings to maintain licensure and/or certification. Bilingual English/Spanish or other non-English language skills.
