Job Description

Gulf Coast Social Services (GCSS) is a private, non-profit corporation founded in 1983 in Louisiana to provide comprehensive human services programs for Louisiana citizens, children, and adults with various physical and mental challenges. Summary We are seeking a dedicated Community Health Worker to join our Baton Rouge team. In this role, you will play a vital part in supporting individuals with disabilities and promoting public health initiatives within the community, aligning with our mission to enhance the quality of life for those we serve. Responsibilities Screen for and assess each family's environmental risk factors and experience of social/environmental adversity that may negatively affect the health and wellness of both children and caregivers. Conduct outreach and education on public health topics within the community. Assist clients in navigating healthcare systems and accessing necessary services. Collaborate with healthcare providers and social service organizations to coordinate care. Facilitate workshops and training sessions on health-related topics. Collect and analyze data to assess community health needs. Advocate for clients' rights and access to services. Manage the distribution of flexible funds. Maintain accurate records of client interactions and progress. Qualifications Bachelor's degree in social work, public health, or related field or five years' relevant experience in community outreach/resource navigation Experience working with individuals with disabilities or in social services (highly preferred). Strong communication skills, both verbal and written. Knowledge of public health principles and practices. Ability to conduct motivational interviewing techniques effectively. Familiarity with data collection methods and medical terminology (preferred). Proficiency in public speaking and community engagement strategies (preferred).
