Valid U.S. driver's license isREQUIRED
Must have reliable access to an INSURED personal vehicle Driving is an essential function of this role (home and community visits required) Applicants WITHOUT a driver's license and vehicle will NOT be considered Must be 21+ and able to pass a driving background check Vinfen is seeking a Community Health Navigator to provide care coordination and connect individuals with medical, behavioral health, and social service supports. This role is part of a multidisciplinary Community Partner Care Team and requires frequent community and home-based visits. Key Responsibilities Conduct outreach and engagement with ACO enrollees Coordinate and complete Comprehensive Assessments (CA) Support development and implementation of Person‑Centered Treatment Plans Connect individuals to community, social service, and healthcare resources Collaborate with PCPs, clinicians, and ACO/MCO teams Support care transitions, including hospital discharges Assist with crisis planning, relapse prevention, and harm reduction strategies Ensure timely and accurate documentation and plan updates Maintain continuity and quality of care across service providers Requirements High school diploma or equivalent required (Bachelor's degree strongly preferred) Minimum of 3 years of care management experience preferred Experience working with individuals with SMI and/or SUD preferred Strong organizational, communication, and documentation skills Ability to work independently and as part of a multidisciplinary team Certified Community Health Worker (CHW) credential preferred Bilingual/bicultural candidates and those with lived experience encouraged to apply Apply today to join a mission‑driven team supporting individuals in navigating care and community resources.Pay:
