Monday-Friday, 9:00 AM-5:00 PM (40 hours/week)

Valid U.S. driver's license is
REQUIRED
Must have reliable access to an OPERATIONAL and INSURED personal vehicle Driving is an essential function of this role (home visits, housing work, community appointments) Applicants WITHOUT a driver's license and vehicle will NOT be considered Must be 21+ and able to pass a driving background check Vinfen is seeking a Community Health Worker (CHW) to provide intensive case management and housing‑stability support through the
CSP/HRSN
programs for individuals with complex medical, behavioral health, and housing needs. Key Responsibilities Complete intakes, assessments, and person‑centered treatment plans Conduct ongoing risk assessments and crisis/safety planning Connect individuals to healthcare, behavioral health, housing, and social services Provide transportation support and accompany members to essential appointments Deliver pre‑tenancy, tenancy transition, and tenancy‑sustaining supports Coordinate care transitions, including hospital discharges Collaborate with PCPs, hospitals, housing providers, and community agencies Maintain accurate documentation for billing and compliance Participate in multidisciplinary team meetings and required trainings Requirements Bachelor's or Master's degree in Human Services, Mental Health, Social Services, or Criminal Justice preferred (experience may substitute) Experience working with individuals with SMI and/or SUD preferred Knowledge of housing resources, Housing First principles, and care coordination Strong organizational, communication, and documentation skills Ability to work independently and as part of a multidisciplinary team Certified Community Health Worker (CHW) preferred Bilingual/bicultural candidates and those with lived experience encouraged to apply Apply today to join a mission‑driven team supporting recovery, housing stability, and long‑term community success.
