MGH Down Syndrome Program:
The Massachusetts General Hospital Down Syndrome Program (MGH DSP) is a collaborative, multidisciplinary team serving people with Down syndrome of all ages and their families. We provide evidence-based clinical care, education, and cutting-edge research so that individuals with Down syndrome can reach their full potential. The Down Syndrome Program is seeking a Community Health Worker to assist our patients with Down syndrome and their families with accessing benefits, navigating assistance programs, and advocating for services. This is a 16 hour (two day) a week position on Tuesdays and Thursdays. This position will be focused on in-office work at our 125 Nashua Street administrative offices. There may be some community based/in-home work as needed, so the CHW may need ability to travel around greater Boston area when in-home support is needed. The office space is located at 125 Nashua Street, Boston, MA. The Down Syndrome Program is a subspecialty service of the Division of Medical Genetics in the Department of Pediatrics (MassGeneral Hospital for Children) at Massachusetts General Hospital. This program offers specialized services for children, young adults, and adults with Down syndrome and their families. Working with direction from the DSP social services team, the successful candidate will support patients with Down syndrome and their families through community resource navigation, benefits access, care coordination, advocacy, and follow-up. Provide community health work services to help patients and families access benefits, public assistance, and other appropriate community-based and hospital-based programs. Assess and address social determinants of health and other barriers to care, including food, housing, transportation, childcare, scheduling, and in-home barriers to follow-up and treatment adherence. Support patients and families with system navigation, resource identification, applications, and referrals for services such as DDS, SNAP, Medical Assistance, Community Servings, MGB ACO resources, behavioral health supports, and other relevant programs with timely and appropriate follow-up support. Partner with patients, families, and providers to identify care goals, reinforce health education, encourage follow-up care and self-management, and help implement practical supports in the home or community environment. Provide advocacy and care coordination across medical, developmental, educational, and community systems, including ancillary appointments, Early Intervention, educational advocacy, and services following a new diagnosis. Work effectively with multidisciplinary providers, interpreters, and care teams to ensure culturally responsive communication and coordinated support for patients and families. Document patient encounters and interventions in the electronic health record, maintain communication with providers, and track progress toward short-term goals and care benchmarks. Identify when additional internal or external care management or specialty supports are needed and facilitate appropriate outreach and referrals.
