The Opportunity:
Whittier Street Health Center is seeking a full-time Maternal Health Community Health Worker (CHW) to support pregnant and parenting individuals throughout the perinatal period. In this role, you will conduct community outreach, provide health education, respond to referrals, offer emotional support, and coordinate care across a multidisciplinary team. This position is ideal for someone passionate about maternal wellness, mental health, and reducing health disparities.
Key Responsibilities:
Conduct outreach in Roxbury, Dorchester, and nearby neighborhoods to engage pregnant and postpartum individuals Build trusting relationships through home visits, phone check-ins, and in-person support Recruit and assist participants in Centering Pregnancy, Centering Parenting, and maternal wellness programs Provide education on pregnancy, postpartum recovery, infant care, breastfeeding, and perinatal mental health Conduct social needs screenings and connect families with resources such as WIC, behavioral health, housing, food support, and early intervention Facilitate peer support groups, workshops, and wellness activities Coordinate care with OB/GYN, Pediatrics, Behavioral Health, Nutrition, Fitness, and community partners Maintain accurate documentation and track outreach, referrals, and outcomes in program databases Represent the program at community events, wellness fairs, and maternal health networks
Qualifications:
High school diploma or equivalent required; Associate's or Bachelor's degree preferred Doula certification or willingness to obtain within 6 months Experience working with pregnant or parenting individuals in urban, multicultural communities Knowledge of maternal and child health, community resources, and trauma-informed care Lived experience as a parent, doula, peer counselor, or caregiver strongly valued Strong communication, organization, and documentation skills Join Our Team!
