Job Description

Outreach Counselor 3.2 3.2 out of 5 stars We are currently seeking an Outreach Counselor for our Care Management CSP team . In this role, the Outreach Counselor will provide direct, community-based support to individuals with behavioral health needs, helping them access resources, build independent living skills, and work toward their personal recovery goals. The Outreach Counselor will collaborate closely with families, providers, and community partners to ensure coordinated, person-centered care. This is an opportunity to make a meaningful impact by empowering individuals through advocacy, engagement, and compassionate support within a collaborative, recovery-oriented team environment. Key Responsibilities Provides in-person, telephonic, and Telehealth care management supports to individuals served based on the collaborative spirit of the integrated health model that promotes improved health, independence, and self-sufficiency. Meets individuals admitted to 24-hour facilities (e.g., hospitals and substance use disorder treatment facilities) in-person or via Telehealth and maintains regular contact with them during their stay to assist with discharge planning and re-engagement with outpatient providers, community services, and social supports. Acts as primary "point of contact" for all individuals served on caseload, ensuring their needs are met. Completes Bio-Psycho-Social Assessment, Individualized Action Plan, Crisis Plan, and Discharge Plan in collaboration with providers and supports. Includes all appropriate people in the individual's care including primary care physician, nurses, mental health and substance use providers, and natural supports as identified by the member, with their written consent. Demonstrates working knowledge of community services and how to help individuals access them including health insurance, food and nutritional assistance, housing placement and stabilization, financial assistance, utility assistance, educational/vocational supports and legal assistance.
What You'll Bring:
High School Diploma or GED required. Community Health Worker Certification and/or Bachelor's Degree is preferred. Bilingual, Spanish and English Speaking is preferred. Knowledge of mental health and substance use disorders, experience with justice involved populations and justice entities, care management services and how to access community resources. 1-2 years of work experience in a related behavioral health, substance use, and/or related human services field is required. Valid driver's license required. Vehicle for use during all working hours
