Job Description

Regular Schedule:
34 hours per week A Non-Merit vacancy exists in Clinical Services/Maternal Child Health, Department of Health and Human Services. Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan. A list of eligible applicants will be established based on the examination as outlined below. Current and future vacancies occurring in any Office or Department of Baltimore County General Government may be filled from the list of eligible applicants. All interested candidates must apply at this time. List all promotions and changes in job duties due to reclassification as separate work experiences on your application. Under general supervision, performs routine community health worker duties for the Babies Born Healthy Program, provides care coordination, case management, conducts virtual and in-home interviews(as needed) and virtual telehealth visits to pregnant and post-partum women, providing care coordination and perinatal navigation services, connecting women to resources, information andand distributes supplies. Provides care coordination, health education, identifies resources, and conducts telephone and in-home perinatal navigation services for pregnant and postpartum women in the program. Conducts interviews and performs screenings for high-risk pregnant and postpartum women to identify needs, link to resources in Baltimore County and provide education on prenatal and postpartum topics. Coordinates with community partners and agencies for available inter-agency community programs and services. Assists and links clients and families with resources. Completes documentation, encounter forms and data tracking. Maintains cooperative and liaison relationships with public and private agencies.

Required Qualifications Possession of a high school diploma or appropriate equivalent plus at least one year experience working with the public. Knowledge, Skills, and Abilities Knowledge of English grammar. Knowledge of and sensitivity to cultural influences. Ability to learn and follow county and departmental rules, regulations, policies, and procedures; ability to establish effective working relationships with clients, families, and public and private agencies; ability to communicate effectively with individuals and groups; ability to work flexible hours; ability to work in a variety of environments; ability to maintain documentation regarding activities; ability to maintain records, files, and reports; ability to coordinate activities; ability to maintain inventory of supplies. Licenses or Certificates Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment. Proof of Licenses, Certifications and Education Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable. EXAMINATION PROCEDURE
Applicants will be qualified based on an evaluation of their training and experience, as stated on their application, which includes answers to the supplemental questions. Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes. Persons appointed to this classification may be required to work evenings and a rare weekend. The work of this classification requires traveling throughout the county, and therefore, requires daily availability of a vehicle. Training for this position will be provided; successful completion of the training is required. Medical Examination and Employment Background Investigation Applicants selected for an appointment to a position in Baltimore County must successfully complete a physical examination and drug screen and an employment background investigation, including, but not limited to a criminal background, education, and fingerprint check.
