Job Description

Upper Great Lakes Family Health (UGLFH) is seeking a Community Health Worker to serve as a liaison between UGLFH patients and health & social services within the community. Responsibilities, included but not limited to: Identifies and tracks patients with barriers to care i.e., transportation, financial, housing/shelter needs, etc. Identifies and tracks patients with health literacy issues and ensures the patient is provided with the appropriate resources. Provides patient education and connects patients to community resources. Partners with Care Managers to provide targeted interventions to avoid hospitalization and emergency department visits. We are designated as a Federally Qualified Health Center (FQHC), providing services regardless of language, gender, socio-economic status, sexual orientation, physical and mental capacity, age, religion, housing status and ability to pay. Our Mission Providing exceptional health care services for all people in the Upper Great Lakes region regardless of their ability to pay. High school diploma or equivalent. Knowledge of community resources and demonstrates knowledge of PCMH model of care. Michigan Community Health Worker Alliance (MiCHWA) CHW Certification or willingness to obtain upon hire. Experience utilizing an electronic health record system. Experience working in a healthcare environment. Willingness to travel occasionally to clinics, as needed.
