(DCHC) is a non-profit community based primary care organization committed to providing accessible, affordable, and quality service to all members of the communities we serve. EOE Bilingual Community Health Worker/Outreach A Growing and Progressive Community Health Center (with six (6) locations throughout the metro Detroit area) has an Immediate Opening for a Full-Time Bilingual Community Health Worker/Outreach for a busy primary care practice in a patient friendly ambulatory setting. Responsibilities Exhibit excellent working relations with patients, visitors, and staff, effectively communicating DCHC's mission. Establish trusting relationships with patients while providing general support and assistance. Greets and register patients in a prompt, pleasant and helpful manner; actively seeks information to understand patient circumstances, problems, expectations and needs. Assist patients in utilizing resources, including scheduling appointments, and assisting with completion of applications and/or registration forms for programs for which they may be eligible (i.e. Medicaid, WIC, etc.) Conduct outreach activities to promote public health initiatives within the community. Provide education and resources to patients, community members, partner organizations, and business to build coverage option awareness regarding health and wellness. Serve as patient advocate and liaison between the patient/family and community service agencies (i.e. schools, support groups, hospital etc.) Collaborate with healthcare professionals to ensure comprehensive care and support. Participate in internal/external committee meetings. Continuously expand knowledge and understanding of community resources, services, and programs through in-service meeting and education programs.
Education :
High School Diploma or GED Completion of a Community Health Worker formal training program (preferred) Qualifications Experience working in a multi-cultural community base setting four (4) years of Health/Social Services experience and/or education Good communication including oral, written, and presentation Bilingual proficiency in Spanish (preferred) Computer Skills required
