Job Description

Position Overview We are seeking a Community Health Liaison also known as a Community Health Worker (CHW) to join our correctional health and reentry services team. The Community Health Liaison will play a vital role in supporting detainees during transition and reentry, coordinating referrals, and strengthening partnerships with community-based organizations. Working closely with Licensed Master Social Workers (LMSWs), nurses, and medical health providers, the Community Health Liaison helps bridge gaps in care and ensure continuity of services post-release. Key Responsibilities Provide case management support, including coordinating referrals, identifying community resources, and following up with clients after release. Assist with transition planning by making direct phone calls, visiting community-based organizations (CBOs), and faxing referrals recommended by CHWs or MSWs. Support education and outreach efforts, offering detainees health, recovery, and social resource information in collaboration with social workers, nurses, and medical/mental health providers. Assist the MSW with implementing individualized transition plans for detainees reentering the community. Coordinate release of information (ROI) requests with the clerk and other team members in accordance with compliance requirements. Collaborate with the Psychiatric Mental Health Nurse Practitioner (PMHNP) consultant to help develop and sustain CBO/DDC partnerships. Assist in prior authorization processes for placements (e.g., residential treatment, detox, respite, or recovery homes) in partnership with social workers, clerks, and healthcare providers. Ideal Candidate Qualities Strong communication and relationship-building skills with both clients and partner organizations. Commitment to advocacy and equity, with a passion for serving justice-involved and underserved populations. Ability to work as part of a multidisciplinary team and independently in the field. Knowledge of community resources and the barriers faced by individuals transitioning from incarceration. Preferred Certification, Credentials, and Experience Certified Peer Recovery Coach or Peer Support Specialist. Experience or training in Behavioral Health or Mental Health First Aid. Lived experience navigating behavioral health, recovery, or justice-involved systems strongly valued. Community Partnership Development The Community Health Liaison will work with a variety of organizations and systems to strengthen referral networks, including: Community Mental Health Service Providers (CMHSPs) Specialty Courts (Drug and Mental Health) Homeless shelters (CAM) Detox and treatment facilities Hospitals and healthcare systems Respite and recovery homes Faith-based organizations and churches Private therapists and psychiatrists Food banks and social activity clubs
