LOCATION
Community Health Awareness Group - Detroit, MI 48226
POSITION SUMMARY:
Under the supervision of the Prevention Coordinator, are responsible for recruiting high-risk persons for HIV, STI and viral hepatitis screenings. The Community Health Worker (CHW) will conduct street and institutional outreach and also serve as a coach for high-risk clients who agree to serve as Recruiters for CHAG's Social Networking Strategy to recruit high-risk persons for targeted HIV testing, screening for syphilis and other STI's, and high-impact prevention (HIP) interventions targeting HIV-positive and high-risk negative interventions. The CHW will conduct the ARTAS intervention with all newly diagnosed HIV-positive persons, addressing barriers to access medical care and coordinate with the Linkage to Care Specialists to ensure linkage to HIV medical care. The CHW will serve as navigator to link HIV-positive and high-risk negative persons with required/recommended supportive services as well as funded HIP interventions. The CHW will screen and refer HIV/STI-positive persons to Partner Services. They will also participate in quality assurance measures.
ESSENTIAL FUNCTIONS
Provide client centered targeted HIV, STI and viral hepatitis screening and testing. Conduct street outreach and social networking recruitment activities. Conducts ARTAS sessions with all newly diagnosed clients which includes strengths-based assessment, care plan, and session notes leading to linkage to medical care. Maintain regular communication with project supervisor and staff. Obtain technical assistance and training as needed and demonstrates knowledge of communicable diseases. Fulfill reporting objectives, as required on event, contact and quarterly reporting forms, in a timely and thorough manner.
EDUCATION AND EXPERIENCE
High school diploma or equivalent. Proven knowledge and understanding of HIV/AIDS, STI's and viral Hepatitis and the psychosocial issues involved in the diseases, substance abuse and other at risk behaviors. Certified State of Michigan HIV rapid tester. Phlebotomy training preferred. Social media and MS Word experience preferred.
JOB PREREQUISITES
Excellent verbal and written communication skills Current Michigan driver's license Must be able to work evenings and weekends as required
