Job Summary Under the guidance and supervision of the Clinical Practice Administrator in coordination with the Quality Improvement Specialist, the Community Health Worker (CHW) is responsible for improving the health and wellbeing of the population served by assisting high-risk individuals and families with access to available community resources and health programs. This position is a dual - role with a primary focus of supporting populations experiencing Social Determinates of Health (SDOH) needs, specifically food and nutrition. The other primary focus will be on Health Navigation and will support patients with Medicaid applications. This position will be co-located between Creston, Wyoming and Streams. The Community Health Worker will be responsible for building relationships with patients and staff, developing integrated workflows that guide patients to community resources, facilitating, scheduling, and other administrative tasks on behalf of and with patients, documenting encounters in the EHR and completing billing requirements. The Community Health Worker functions as a centralized resource that educates and informs staff about community resources. Additionally, the position acts as a liaison for the Greater Grand Rapids area by interacting with other health care centers and resource centers to offer coordination of patient care and to connect patients to resources that address the social determinants of health.

Clinical Practice Administrator for Creston & Wyoming Description of Duties/Responsibilities -
- Document patient encounters in the EHR and completes required billing for CHW services
- Follow-up on patient cases in the EHR system for all referred patients
- Monitor CHW inbox in EHR
- Conduct Medicaid outreach, enrollment and redetermination.
- Attend relevant meetings as directed.
- Attends daily huddles at assigned work sites.
- Link patients to community resources and follow up on the outcomes of those referrals.
- Build strong working relationships with community support networks.
- Explore and evaluate new programs, services and resources for quality and requirements.
§ Connect patients with outside resources to address SDOH (i.e. transportation, clothing, shelter, food, etc.) § Meet with clients to address identified needs through community resource coordination. § Offer scheduled appointments to meet with assigned patients to assess needs of the individual and offer a service specifically designed for their unique needs § Collaborate with the School-Based Community Health worker and Homeless Health Case manager § Support Administrative coordination of CHC health navigation status and communicate federal and state updates § Develop and maintain professional relationships with other health care organizations and community partners § Update and maintains SDOH SharePoint § Perform other related duties as assigned Minimum Position Qualifications § High School Diploma or GED § Certification as a Community Health Worker § Training/Experience in Health Navigation preferred § Two years' experience in medical office or similar setting preferred § Lived Experience preferred § College education in social work, community engagement, or public health preferred Skills, Knowledge, and Experience Requirements § Effective verbal, written and interpersonal communication skills with the ability to comfortably interact with diverse populations, including differing attitudes, personalities, values, and beliefs § Core Competencies for
