Community Health Worker (CHW) Join the LifeWays team and help connect individuals and families to the care, resources, and support they need to thrive. Under the direction of the Supervisor of Community Health & Wellness, the Community Health Worker (CHW) serves as a trusted link between the community and health/social service systems, helping improve access to care, reduce barriers, and promote whole-person wellness. What Youll Do Support individuals through in-office, in-home, and community-based outreachConnect consumers to community resources, benefits, and support servicesConduct Social Determinants of Health (SDOH) screenings and follow-up supportAssist with appointment scheduling, reminder calls, and wellness program participationAdvocate for individuals and help improve access to behavioral health, substance use, and physical health servicesProvide encouragement, education, and recovery-focused supportFacilitate communication between consumers and healthcare/social service providersAssist with wellness groups, health monitoring, and care coordination activities
