Housing Matters Supportive Housing Program, a Housing First agency, offers supportive housing case management and advocacy for long-term homeless adults, youth and families living in Beltrami County. We provide housing navigation, in home case management services, mental health, SUD services, and Community Health Services in an effort to provide a robust wrap around approach to meet the distinct needs of the people we serve. It is our mission at Housing Matters that "All people, regardless of where they are on their journey have an inherent right to safe, stable and affordable housing." The Community Health Worker is ( CHW ) is a trained health educator who works with our participants who may have difficulty understanding providers due to cultural or language barriers. CHWs work with underserved communities, reduce health disparities, enhance provider communication, and improve health outcomes and overall quality measures. Working in conjunction with primary care providers, CHWs can bridge gaps in communication and instill lasting health knowledge. This position focuses on improving the health status and social drivers of our participants, both individually and as a community. The Community Health Worker (CHW) is responsible for primarily working in paticpants' homes and in the community. CHWs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve. CHWs bridge cultural and linguistic barriers, assist with access to care, address social drivers of health, and improve health outcomes. CHWs work to reduce health inequities, boost cultural competence, and empower individuals and communities to achieve better health through advocacy, education, and support. The CHW works closely with medical and behavioral health providers, primary care teams, care coordinators, county partners, and other agencies to improve overall care and outcomes. This position works both in office and in people's homes and their community in order to carry out their job responsibilities.
Job Duties:
Carry a caseload of assigned clients and meeting with them at regular intervals. Identifying needs and individualizing services. Conduct 1:1 meetings in home or community settings. Develop appropriate curriculum and conduct weekly group meetings with participants in our program. Help participants overcome barriers to care. Coach and motivate participants to be active and engaged in their health, overcoming barriers to obtaining needed medical care and social services. Document activities, service plans, and results in an effective manner. Establish positive, supportive relationships with participants. Help participants access services, including scheduling appointments, establishing appropriate medical providers, accompany participants to provider visits and appointments with community resource agencies; and assist with completion of applications for programs for which they may be eligible. Build and maintain positive working relationships with providers, case managers, agencies, etc. from diverse backgrounds in order to reduce cultural and socio-economic barriers between members and institutions Provide social support and informal counseling. Create connections and help manage care transitions between vulnerable populations and health care providers. Reduce social isolation among our participants; provide social support, informal counseling, health screenings, and referrals. Promote cultural competence among health care providers serving vulnerable populations Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition. Other Requirements:
Candidate must possess a Community Health Worker Certificate from a MNSCU college/technical program. Valid driver's license with clean driving history. Written consent to a DHS background check, and ability to pass this is contingent on being offered employment with our agency. Additional Information:
Be able to successfully build healthy working relationships with all participants through strength-based recognition and a client driven approach to providing services. Assist participants to build upon their strengths and recognize their needs through respect and consideration of their specific situations. Keep accurate and up to date information on all participants including but not limited to goal plans, case documentation, and other information as needed. Work as part of a team to plan and develop resident programs. Attend regular internal and external agency meetings. Attend trainings that increase knowledge of effectively meeting participant needs. Work collaboratively and as a team member and within the organization to implement the mission and plans of Housing First, Beltrami County, MN DHS, and Housing Matters. Job Type:
