As a Navigator in the Participant Wellness & Safety Program , you will work directly with individuals experiencing homelessness, substance use disorders, and complex health challenges. Through trust‑based relationships and low‑barrier engagement, you will connect participants to critical services like healthcare, treatment, harm reduction resources, and housing pathways. This role is ideal for someone who is passionate about meeting people where they are , building relationships without judgment, and helping individuals move toward stability, health, and recovery. What You'll Do Build trusting, consistent relationships with participants using trauma‑informed, person‑centered approaches Connect individuals to healthcare, MAT/MOUD, harm reduction resources (including Naloxone), and treatment services Support participants in developing and working toward self‑directed recovery and wellness goals Coordinate referrals and "warm handoffs" with internal teams and community partners Conduct outreach and engagement with individuals experiencing homelessness, including those not connected to services Host or support HIV and Hepatitis C testing events and connect participants to follow‑up care Maintain accurate and timely documentation in electronic health record systems and required reporting tools Develop crisis plans and provide de‑escalation and crisis intervention as needed Collaborate across teams and partners to improve access to services and reduce barriers Contribute to a culture of dignity, safety, and equity in all participant interactions What You Bring High school diploma or GED At least 2 years of experience working with individuals experiencing homelessness or similar populations Experience supporting individuals with substance use disorders, mental health needs, or complex barriers Experience using electronic health records and tracking participant data Strong relationship‑building skills with the ability to engage participants in a non‑judgmental, person‑centered way Understanding of trauma‑informed care, harm reduction, and recovery‑oriented approaches (or willingness to learn) Knowledge of community resources, housing systems, and behavioral health supports Ability to manage documentation, communication, and multiple priorities in a fast‑paced environment Strong communication and collaboration skills Lived experience is valued and may be considered as part of experience requirements Valid driver's license, reliable transportation, and ability to travel within the community What We Offer 32‑hour full‑time work week Medical Health plan Health Savings Account (HSA)
