Job Description

Knowledge, Skills, and Abilities Knowledge of infectious diseases, including HIV, sexually transmitted infections (STIs), Hepatitis, and other communicable diseases. Understanding of public health principles, prevention strategies, and health promotion practices. Familiarity with community resources, social service agencies, and healthcare systems that support underserved populations. Skills in written and oral English. Ability to work in a multi-cultural setting. Knowledge of basic medical terminology. Knowledge of basic computer skills. Strong communication and interpersonal skills with the ability to engage individuals from diverse backgrounds. Effective health education and outreach skills to promote disease prevention and healthy behaviors. Ability to initiate and maintain a positive working relationship with CFHC staff and other organizations. Understanding of the community served - community connectedness. Ability to communicate efficiently, including verbal and listening skills. The ability and willingness to provide emotional support, encouragement and motivation to patients and families. Skills in basic reporting and tracking of patient services through manual and electronic processes. The ability to travel for job duties and trainings. Qualifications/Education Requirements High school diploma or GED required; associate or bachelor's degree in public health, social work, or a related field preferred. Successful completion of a Community Health Worker formal training program, such as from a college or other education institution, is preferred. Applicants must have basic computer skills. Experience working in a community-based setting for at least 1 to 2 years preferred. A valid driver's license and reliable transportation are necessary. The employee must provide proof of minimal liability coverage as required by the state and will be required to provide updates when renewed. Conduct intake assessments with patients and promotes referral for services into Coastal Family Health Center, its programs, and other available community resources as appropriate. Provides ongoing follow-up visits utilizing basic motivational interviewing with patients and families. Follows up with patients through phone calls, home visits and community events (health fairs, community forums, advocacy meetings and workshops). Promotes disease prevention strategies and supports community health education initiatives. Assist patients with completing applications and registration forms for various patient services. Assist patients in setting personal healthcare goals. Assist patients in connecting with transportation resources. Exhibits excellent working relations with patients, visitors and staff, effectively communicating the mission of Coastal Family Health Center (CFHC). Works closely with medical provider and other clinical personnel assigned to the care team to help ensure that patients have comprehensive and coordinated care. Acts as patient advocate and liaison between the patient/family and community service agencies (i.e. Department of Health and Human Services, hospitals, support groups, etc.). Documents patient encounters within the Electronic Health Records (EHR) as well as additional databases, as necessary, utilizing approved workflow processes. Attend regular staff meetings, trainings and other meetings as requested. Manages assigned caseload of patients. Upholds complies with and enforces the Core Principles and Code of Conduct. Perform other duties as assigned. This is a 2-year grant position.
