Job Description

Community Health Worker - New You! Reports to Director of Programs
MINIMUM REQUIRMENTS
Minimum of AA degree. Two (2) years' experience in a related field Two (2) years or more as resident in Lafayette and surrounding Counties and working knowledge of community resources and events. Proficient computers skills, including Microsoft, specifically Word and Excel, in addition to excellent oral and written communication skills. Must have a valid Mississippi Driver's License, proof of insurance and reliable transportation
RESPONSIBILITIES
Conduct individual home visits with the participants working on losing weight and changing mindset. Manage caseload (up to 75), keeping accurate records and ensuring that all related administrative tasks are completed and up to date. Provide the evidence-based curriculum and emotional support. Maintain an effective support service for participants with the aim of enabling participants to lose weight and helping with any resources necessary to help their develop of a healthier lifestyle. Design and participate in recruitment activities especially promoting increased awareness of services by educating the public and linking with other programs. Maintain confidentiality of all participants; communicate orally and in writing to parents, professionals, and other agencies. Plan and implement activities and educational classes for participant that are culturally sensitive and developmentally appropriate both in individual and group settings. Other duties as assigned.
