Job Description
Join a dedicated team working to improve health outcomes by addressing the social factors that impact well-being. As a Community Health Worker, you will support patients and families through care coordination, home visits, resource navigation, and advocacy. Working closely with healthcare providers and community organizations, you will help individuals access housing, transportation, insurance, food resources, and other essential services while empowering them to achieve their health and wellness goals. This role is ideal for a compassionate, resourceful professional who is passionate about making a meaningful difference in the community.
KNOWLEDGE/EXPERIENCE
Knowledge of Helena community resources and experience in navigating community resources. Lived experience (ie. Previous homelessness / housing crisis, social determents of health, social support (SNAP, TANF, etc.) is preferred but not required. Previous experience as a CNA, CMA, or role in a social support office preferred but not required.
EDUCATION
High school diploma or equivalent preferred. Proof of previous completion of a Community Health Worker training course, or completion of the Community Health Worker training course within six months of hire.
LICENSE/CERTIFICATION/REGISTRY
BLS Required. MT Driver's License, car insurance, and vehicle (mileage reimbursed).
